Traces of LNT are found in the milk of other mammals, but it’s massively present in human milk. It’s of such great importance for a healthy human microbiome that it’s believed to be the result of highly specialized coevolution of the microbiome and human milk1.

LNT represents about 9% of the HMOs in mothers’ milk. As well, it’s a core structure from which many other HMOs are built from. This is unique to humans.

ABOUT HMOs: The third largest component of mothers’ milk solids, HMOs are unlike regular sugars and don’t provide energy to the infant. Rather, they are prebiotics that feed an infant’s microbiota. They are also bioactive and deliver direct health benefits on their own. Each mother’s milk has a unique combination of HMOs, dependent on genetics, nutrition, lactational stage, mother’s lifestyle, geography and more2.

GUARDS AGAINST UNWANTED MICROBES
HMOs have multiple methods to rid the body of unwanted microorganisms, such as acting as decoys or altering epithelial cell gene expression3, 4, 5. Each HMO has a highly specific mode of action6.

ESTABLISHES AN OPTIMAL GUT MICROBIOME

HMOs are prebiotics that promote beneficial bacteria growth4, 7. Long known to attract the healthy Bifidobacteria8, 9, recent studies highlight that over 100 non-Bifidobacterium species also use HMOs for their growth10. These bacteria then thrive and produce the metabolites necessary to support good health9.

Among all known HMOs, LNT appears to exhibit the strongest prebiotic effect. It’s been hypothesized the widespread microbial utilization of LNT reflects a co-evolutionary relationship between human milk composition and microbiome development in early life. Many bifidobacteria have evolved the ability to metabolize LNT, including infant-type species such as B. breve, B. infantis, and B. pseudocatenulatum, as well as other species including B. longum longum, B. scardovii, and partially B. catenulatum. Microbes ferment LNT and produce lactate and short-chain fatty acids. These metabolites lower the intestinal pH and create an environment that supports a bifidobacteria-dominated microbiota and suppresses opportunistic microbes1, 11, 12.

PROMOTES A HEALTHY GUT
HMOs may promote mucus production13, thereby supporting the physical barrier between the intestinal epithelium and gut content. As well, metabolites produced by HMO fermentation, like short-chain fatty acids, can modulate the immune system and strengthen the gut barrier14.

LNT promotes growth of specific Bifidobacterium strains. They then produce metabolites like acetate, which contribute to strengthening the gut barrier15. Beyond its prebiotic effects, LNT has also been shown to directly support intestinal barrier integrity16.

MATURES THE IMMUNE SYSTEM
HMOs interact with immune cells both locally in the intestine and systemically, supporting immune cell balance and immune signaling pathways. Their effects are mediated both via direct interactions with immune cells and via metabolites generated during HMO fermentation.

LNT appears to modulate the immune system indirectly. Current evidence suggests that LNT does not interact directly with immune cells. Instead, its effects are likely mediated through prebiotic mechanisms.

A range of Bifidobacterium strains can utilize LNT as a substrate. During fermentation, these bacteria convert LNT into several metabolites, including short-chain fatty acids (SCFAs) and aromatic lactic acids. Both SCFAs and aromatic lactic acids are known to have immunomodulatory properties. They are also considered important contributors to immune system development during early life.

SUPPORTS BRAIN DEVELOPMENT AND COGNITION
HMOs are a source of sialic acid and fucose, both of which are essential components for brain development. They also play key roles in neurological processes. A small proportion of certain HMOs can enter the bloodstream, where they may exert positive systemic effects. In addition, HMOs are fermented into microbial metabolites that influence the microbiota–gut–brain axis.

LNT is rarely associated with cognitive outcomes. This aligns with its primary role in modulating the microbiome and supporting gut health17.

PERFORMANCE IN ADULTS
LNT has not yet been studied in adult populations; however, clinical research is currently underway.

Importantly, the current lack of published data does not imply a lack of potential benefit. Novonesis is actively investigating the health effects of LNT across different age groups.

Many of the underlying mechanisms observed in infants particularly those related to immune and microbiome modulation as well as gut health, are also relevant in later life stages. Further data are expected in the future.

See what science tells us

HMOs represent the beginning of complex processes where it's difficult to determine cause and effect. Much more research is needed to clarify their individual and collective benefits. Here’s a summary of what we know now.

More about HMO science

Breastmilk is the gold standard in infant nutrition. A major difference between milk from humans and other mammals is the amount and abundance of oligosaccharides. Back in 2015, when we were the first to make HMOs commercially available with our MyOli ® brand, HMOs were mainly known for their prebiotic benefit.

  • HMOs are the third largest component of breast milk

    Breast milk nourishes baby’s microbiome

Mother’s milk has evolved to give a baby and its microbiome a good start in life. Human milk oligosaccharides can’t be digested by the body— only by bacteria in microbiome.

HMOs are the third largest solid component.

  • The three HMO classes

Over 200 HMOs belong mainly to three classes

The vast majority of HMOs belong to one of three classes. 70% contain fucose and are called Fucosylated HMOs. Sialylated and neutral HMOs split the remaining 30%. SL HMOs contain sialic acid. Neutral HMOs have neither fucose or sialic acid, and are building blocks for more complex molecules4.

  • Health effects of HMOs

Three HMO classes with many health benefits

Each HMO is unique and offers multiple benefits. In general, FL HMOs tend to be good at protecting against unwanted microbes. The sialic acid in SL HMOs is important to brain and cognitive function. Neutral HMOs nurture a healthy microbiome18. This is just the tip of the iceberg for HMO health benefits.
